It works ok here, but I saw the:

RG> OUT &H3CE, 4: OUT &H3CF, 2: BSAVE "plane3", 0, 38400
RG> OUT &H3CE, 4: OUT &H3CF, 3: BSAVE "plane4", 0, 38400

added a ".BAS" extension to the files (in the QB4.5
environment) Perhaps a file name conflict occured
somewhere...

P.S. Does he have a super EGA card (w/ bios support for VGA
16 color modes?  They made sever such cards back when VGA
was new)
